Please make your registration on-line or fill in the registration form (PDF) and return it by mail or fax to the address below. The registration form is also to be used for hotel reservations. The registration fee entitles you to:
Attendance at all scientific sessions, exhibition, lunch, coffee, Final Programme, abstract book, free acces to the online version of the symposium proceedings for six months, travel card for public transportation during the Symposium, attendance at the Opening Ceremony and Welcome Reception at the Stockholm International Fairs on June 26 at 19.00, a Reception at the Stockholm City Hall by invitation of the City of Stockholm and the County Council on June 27, at 18.30, and Farewell Reception.

Accompanying person fee entitles you to:
Opening Ceremony and Welcome Reception at the Stockholm International Fairs on June 26 at 19.00, a Reception at the Stockholm City Hall by invitation of the City of Stockholm and the County Council on June 27 at 18.30 and Farewell Reception.

Processing and Publication of Personal Data
The personal information we receive from delegates will also be required after the Symposium. This is in order to render an account of the conclusions arrived at during the Symposium, for statistical purposes, and for the dispatch of information on relevant symposia in the future. Personal data will also, in certain cases, be released to other companies participating in the Symposium for the distribution of information and offers directly from them. If you do not wish your personal data to be published, please inform us of this fact when you register for the Symposium or at the latest on May 25, 2005. Those participating in the actual running and delivery of the Symposium (lecturers, moderators and others) are assumed to have given permission for the above.

Confirmation
A confirmation letter will be sent to you well in advance of the Symposium, confirming your registration, booked events, hotel accommodation and your payment status.

Registration and fees
According to Sixth VAT Directive, 77/388/EEG, Article 9.2.(e), participants from different countries should pay their registration fee either including or excluding Swedish VAT 25%. Therefore, please identify your category below and choose the appropriate alternative in the registration form:
